NAIA Official Records Submission Form

Use this form to submit a record from any NAIA-sanctioned sport for consideration as an NAIA national record. Remember, the record must have occured against a school that is:
1) four-year or upper-level,
2) accredited,
3) degree-granting.

You must submit a completed copy of this form plus upload supporting evidence of the record (i.e., an official game box for a game record; game-by-game statistics for a season record; year-by-year statistics for a career record). Also use this form to submit career totals for a Top 10 ranking (i.e., career scoring, career touchdowns, career goals, etc.).
